Two (2) identical photos must be submitted with the passport application.
The photos must be no older than 6 months.
No, the minor child must be the only subject in the photo. Nothing used to support the minor child, whether by mechanical or human means, should be in the camera's frame.

* The rule of thumb for men is that many times, where cocktail attire is specified they also require that men wear dinner jackets, which is why men are commonly seen wearing suits. If a dress shirt is worn that does not require a tie, then it is fine not to wear one, however, if a button down shirt is worn with the suit, then a tie would be in good taste.

Semi-Formal Attire

This form of attire gets the most attention, but often because of its universal use. Many associate other forms of dress such as cocktail attire, with semi-formal. However, Semi-formal attire is much more formal than cocktail attire.
